Improving the health of Lane County
through community partnerships


100% Health Coalition

The 100% Health Community Coalition formed as a result of the United Way of Lane County’s 2004 Community Needs Assessment which found that for the first time, health care, affordable medications, and health insurance were the top three needs of people in Lane County regardless of socioeconomic status, age, or geography. The Coalition is comprised of Lane County's leading health partners, including our hospitals, medical practitioners, safety net clinics, insurers, government agencies, private businesses, and non-profit organizations who are committed to improving the health of the community.

The Coalition works to connect people to existing health resources while working to refine the current health delivery system. It also works to develop and implement new community-based medical programs and services to fill the gaps where it is most needed. The Coalition focus has shifted from just access to health care to include the broader social determinants of health. Learn more about the 100% Health Coalition.

Live Healthy Lane

Live Healthy Lane is a collaborative project between United Way of Lane County, Lane County Public Health, PeaceHealth, and Trillium Community Health Plan that consists of a Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP).  The goal of this work is to improve the Lane County community's mental, physical, social and economic health. Reducing health disparities, promoting health equity, and improving overall population health is the central purpose of the health assessment and health improvement planning effort. Community Health Resources:

Low-Cost Medical and Dental Clinics

Oregon’s health system transformation and implementation of the Affordable Care Act are moving our region in the right direction. Despite the advances and the increasing number of people who are covered, there are still many uninsured and under-insured individuals. The Safety Net includes the organizations that provide health care opportunities for those who otherwise would have barriers. These barriers include lack of coverage, geographic isolation, language and culture, mental illness and homelessness. Prescription Savings

FamilyWize has partnered with United Way to provide the FamilyWize Prescription Savings Card. FamilyWize is a Community Service Partnership focused on improving the health of individuals, families, and communities by helping to make prescription medications more affordable.
