How the Community Health Improvement Plan is transforming tomorrow.

When it comes to promoting health, the work happening with Live Healthy Lane is leading the way. Live Healthy Lane is a community-based effort to improve the health and well-being of those who live, learn, work, and play in the Lane County Region.

Together, the 100% Health Community Coalition, United Way of Lane County, Lane County Public Health, PeaceHealth, Trillium Community Health Plan, and numerous cross-sector community partners are working together to improve the health of the community. This collaborative effort consists of the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP).  Reducing health disparities, promoting health equity and improving overall population health is the central purpose of this work.

“We know that health is more than healthcare, it’s a sense of community, access to opportunities, and the best possible start for Lane County’s future generations. With Live Healthy Lane, we’ve put community in the center of our Community Health Improvement Plan,” says Heather Amrhein, Director of Health at United Way of Lane County.

The Lane County region has a strong foundation for a healthy community. Our community is built around abundant natural resources, collaboration across organizations, hardworking residents, caring neighborhoods, and innovation. While this provides a solid foundation for our community, there is still much work to be done.

By working together, we can create a caring community where all people can live a healthier life. The Live Healthy Lane vision unites United Way, Lane County, PeaceHealth, Trillium, local organizations, and YOU to contribute in unique ways to help improve the lives of everyone in the community. Together we can do more than each of us can do alone.
